Registering with the surgery
We are using a new online service called Register with a GP surgery that makes it quick and easy to register with this practice.
The service is designed and run by the NHS, so your personal information is safe and secure. It cuts our workload and makes it easier for you to register online.
You do not need proof of address, immigration status, ID or an NHS number.
If you have an NHS number, you do not need this to hand, but it could make registration easier. You can use the NHS website to find your NHS number.
Non-urgent advice: Register as a New Patient
Just fill in this quick online form to start the process:
Paper forms are still available at reception if you need one.
Once you have submitted your registration (either online or in person) it can take up to 10 working days (2 weeks) for you to become registered with our surgery and for your medical records to be transferred. This can be longer during busy periods.
When you have been successfully registered, patients over the age of 18 will be asked to attend for a New Patient Check with the Practice Nurse.
Named GP
Every patient has a named, accountable GP. They are the main doctor responsible for your care. At this practice, your named GP will be Dr M T Khan.
We do not have any other GPs available to be your named GP. We are a single-handed practice, which means we only have one main doctor.
